What Cervical Radiculopathy Actually Means
When a nerve root in the cervical spine — the part of the spine in your neck — becomes compressed or irritated, it can send pain, numbness, tingling, or weakness along the path that nerve serves. Because cervical nerve roots supply sensation and motor function to the shoulders, arms, and hands, the symptoms often appear entirely in the extremity rather than in the neck itself. A patient comes in describing numbness in the ring and little finger, and the source may be a nerve root at the C8 level in the neck, not anything in the hand or elbow.
This pattern — symptoms presenting away from their actual origin — is what makes cervical radiculopathy easy to misidentify. It can look like carpal tunnel syndrome, thoracic outlet syndrome, a rotator cuff problem, or simply poor circulation. The distinction is not academic — it determines where treatment should be directed.
Common Causes in the Neck
The cervical nerve roots exit the spinal canal through openings called foramina on each side of the vertebrae. Two main processes can narrow these openings and put pressure on the nerve. The first is disc herniation — when the soft inner material of an intervertebral disc pushes outward and contacts the nerve root directly. This can happen acutely after a sudden load, or gradually over time from accumulated disc stress. The second is foraminal stenosis from bone spur formation, where the gradual deposition of bone around the facet joints or vertebral endplates slowly reduces the available space for the nerve. Both are common in adults over 40, and both produce similar symptom patterns.

Extended periods behind the wheel with the head held in a fixed position, combined with road vibration, loads the cervical spine in ways that contribute to disc wear over years. Recognizing the Symptom Pattern
Cervical radiculopathy produces a characteristic dermatomal pattern — symptoms that follow the territory of a specific nerve root. C6 involvement often produces numbness or tingling in the thumb and index finger, with possible pain radiating down the outer forearm. C7 affects the middle finger and can produce weakness in the triceps. C8 and T1 tend to affect the ring and little fingers and the intrinsic muscles of the hand, sometimes producing a grip that feels weaker than usual.
The neck itself may or may not hurt. Some patients have a stiff or sore neck alongside the arm symptoms; others feel nothing in the neck at all and are surprised when an examination points there as the source. Symptoms can worsen with movements that narrow the foramen — extending the neck, turning the head toward the affected side — and improve with positions that open the foramen, such as resting the arm elevated on top of the head. Patients often discover this position on their own before being told it has a clinical name.
How It Is Evaluated
A thorough clinical examination for suspected cervical radiculopathy includes a history of symptom onset, specific location of numbness or weakness, and what positions or activities make it better or worse. Physical examination includes cervical range of motion, upper extremity neurological testing covering reflexes, sensation, and grip strength, and orthopedic provocation tests. Spurling's test compresses the nerve root and can reproduce symptoms; the shoulder abduction relief test reduces them by changing foraminal dimensions.
Imaging such as X-ray or MRI is sometimes appropriate to confirm the level of involvement and rule out other structural causes, particularly if symptoms are severe, progressing, or accompanied by significant weakness. In many cases, the clinical picture is clear enough to begin conservative care before imaging, using imaging to guide decisions only if the initial response to treatment is slow.
Chiropractic Care for Cervical Radiculopathy
Conservative management is the appropriate first approach for the majority of cervical radiculopathy cases. Chiropractic care focuses on reducing the mechanical load on the compressed nerve root through cervical joint mobilization or adjustment, which helps restore normal movement to joints that have become restricted and reduces the compressive forces on the foramen. Many patients with cervical radiculopathy experience meaningful improvement with conservative care over weeks to months, depending on the severity of compression and how long it has been present. Nerve symptoms — numbness and tingling — often improve more slowly than pain, which is normal and reflects the time it takes for nerve tissue to recover. In cases with progressive or severe weakness, or where conservative care does not produce adequate improvement, referral for further evaluation is the appropriate next step.
What to Watch For
Most cervical radiculopathy is uncomfortable but not dangerous. However, certain presentations indicate a more urgent need for evaluation. Weakness that is progressing — losing the ability to grip, lift the arm, or perform fine motor tasks — should be assessed promptly. Symptoms involving both arms simultaneously, difficulty walking, or bowel or bladder changes alongside arm symptoms suggest possible spinal cord involvement rather than a single nerve root, and require immediate evaluation.
Symptoms that begin following a trauma — a car accident, a fall, or a direct blow to the head or neck — should also be evaluated promptly, both to rule out fracture or instability and to begin care before the nerve irritation becomes chronic.
